How do I create my own assessment?

Clovi allows you to create custom assessments (questionnaires) that participants complete during registration. You can use assessments for health risk questionnaires, lifestyle surveys, medical history intake, or any custom data collection need.

Steps

1. Navigate to Account in the top navigation bar
2. Click “Assessments” in the left sidebar menu
3. Click the “Add an Assessment” button
4. Give your assessment a name
5. Add sections to organize your questions (e.g., “Lifestyle”, “Medical History”)
6. Add questions to each section — supported question types include multiple choice, text input, yes/no, and scale
7. Configure question settings such as required/optional and conditional logic
8. Save and assign the assessment to events as needed

Once created, you can assign the assessment to any event. Participant responses are stored and can be exported.
